Ramp door failure on Roll-on/Roll-off (Ro-Ro) vessels can directly affect loading and unloading performance as well as overall operational efficiency. This study aims to analyze the impact of ramp door damage on loading and unloading performance on the Ro-Ro vessel KM Mutiara Ferindo VII. A descriptive qualitative method with a case study approach was employed, using data collected through direct field observation, interviews with the master, chief officer, and bosun, and supporting documentation. The findings indicate that ramp door damage, caused by wire rope failure due to corrosion and insufficient routine maintenance, resulted in the suspension of loading and unloading activities, operational delays of approximately 4–5 hours, and decreased operational efficiency and user confidence. In addition to operational impacts, the incident also posed potential occupational safety risks in the port area. Corrective actions included temporary suspension of operations, area securing, initial inspections, emergency repairs, and evaluation of the vessel’s maintenance system. This study highlights the importance of implementing regular preventive maintenance of ramp doors to ensure loading and unloading performance and operational safety on Ro-Ro vessels.
