Occupational safety aboard ships depends critically on both the availability of and adherence to personal protective equipment (PPE). This study analyzes how to optimize the use of safety equipment to prevent accidents on board MV Sangiang and proposes managerial actions to strengthen crew discipline. A qualitative descriptive approach was employed through interviews, observations, and document analysis involving selected officers and crew members. Findings reveal gaps between procedures and on-deck practice: two recorded incidents—eye irritation due to the absence of standard eye protection and a finger injury from not wearing appropriate gloves—indicate low PPE compliance, weak enforcement of discipline, and insufficient routine supervision. The analysis underscores that equipment adequacy, the effectiveness of safety management (including regular safety meetings), human resource quality, and procedural adherence are key determinants of accident prevention. Recommended improvements include task familiarization and work briefings, consistent officer supervision, enforcement of sanctions and recognition tied to discipline, and timely renewal of PPE to meet standards. These insights are expected to inform efforts to strengthen safety culture on similar passenger vessels.
